We are looking for individuals to join Fairlop PCN who will coordinate the work of healthcare professionals and non-clinical staff including volunteers involved in the care of patients registered at GP practices within the wider PCN population. The successful candidate will be part of a dynamic and forward-thinking PCN led Multi-Disciplinary Team (MDT). A key part of the role of a care coordinator role is in the Care Homes MDT. The successful candidate will act as a point of contact for residents, families and professionals who visit care homes, such as MDT members and in-reach specialists. Support patients to utilise decision aids in preparation for a shared decision-making conversation; Holistically bring together all of a persons identified care and support needs, and explore options to meet these within a single personalised care and support plan (PCSP); Help people to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care; Support people to take up training and employment, and to access appropriate benefits where eligible; Support people to understand their level of knowledge, skills and confidence (their Activation level) when engaging with their health and wellbeing, including through the use of the Patient Activation Measure;

We are in the very fortunate position where all 4 of the Primary Care Networks (PCNs) in Redbridge are working together as part of the Redbridge-wide GP federation Healthbridge Direct.
This means we are able to build strong clinical teams within and across the PCNs. We are now at the very start of an exciting journey of building a team of over 100 additional clinical roles to support and enable the delivery of services within each of our 4 PCNs. We are building teams within and across the PCNs to enable peer support and team working both within and across professions.
Job responsibilities
To play a pivotal role in the delivery of high-quality primary health care within the Primary Care Network
To support the PCN Clinical Directors in achieving the DES specifications
To work closely with PCN Practices and the multidisciplinary team
Support Clinical Directors in coordinating all key activities including access to services, advice and information, and ensuring health and care planning is timely, efficient and patient-centred.
The post holder will help patients interact and engage in everyday life. You will do this through activities designed to develop, maintain or retrain skills for people with a disorder, condition or illness (whether that be cognitive, physical or mental).

Qualifications
* Educated to GCSE or equivalent NVQ 3 or equivalent and/or relevant basic/first level professional qualification
* Experience of working within a social care and/or health setting, working with multi-disciplinary teams. Demonstrable experience of effective planning and organisational skills to deliver targets to deadlines Experience of working with people with diverse health and social care needs Patient clinical system knowledge Experience of using recognised assessment tools Experience of mental health either as service user, carer, worker or volunteer Experience of multi-agency working and signposting to appropriate support
