Job Summary

This position will ensure that the health and safety of all residents are met at Traditions of Preston. The LPN will assist the RN with planning, coordinating, and managing the assisted living services and act within the Assisted living community regulations. The LPN is responsible for the overall direction, coordination, and monitoring of assisted living service delivery under the direction of the RN.

Responsibilities

* Resident Care & Clinical Support
o Provide nursing support in accordance with Assisted Living regulations and community standards.
o Monitor resident health, update care plans, and recommend alternative placement when necessary.
o Assist with medication management: fill pillboxes, monitor renewals, and coordinate with pharmacies and physicians.
o Communicate and document changes in resident condition clearly and promptly.
* Compliance & Documentation
o Maintain accurate and up-to-date resident records and nursing notes.
o Support regulatory compliance efforts; assist in preparation for state licensing surveys.
o Ensure privacy and confidentiality of resident information and living environments.
* Team Collaboration & Communication
o Work closely with RNs and care staff to deliver high-quality, resident-centered care.
o Share key updates through communication tools (e.g., staff communication book).
o Participate in team meetings and contribute to ongoing service improvements.
o Maintain open, respectful communication with residents, families, and team members.
* General Responsibilities
o Carry a cell phone and be available for emergencies or staffing updates.
o Respond to resident concerns and complaints with professionalism and compassion.
o Support events, trainings, and family functions as needed.
o Uphold safety standards and report incidents or hazards promptly.

Qualifications

* Must be licensed with the state as an LPN, and must maintain current and unencumbered licensure.
* Must have a valid driver's license, if driving is required.
* At least 3 years experience in related field.
* Possess a sensitivity and aptitude for working with the elderly.
* Possess skills to communicate effectively with those you supervise, residents, family members and RN Supervisor.
* Possess leadership / supervisory skills.
* Must be able to prioritize and organize work effectively and efficiently.
* Able to work irregular hours, occasional weekends and evenings.
