Care Co-ordinator (Maternity Cover; 9-12 months FTC)

Job Role & Purpose

Aspiro Healthcare is looking for a motivated and compassionate Care Co-ordinator to join our CommunitySupport Team (CST) on a fixed-term maternity cover basis.

This is a key primary care role supporting patients with long-term conditions, frailty, or complex needs. You will work across GP practices and partner organisations to help coordinate care, improve patient outcomes, and reduce avoidable hospital admissions.

You will act as a central point of contact for patients, carers, and professionals, ensuring care plans are implemented effectively and support is joined‑up and timely.

Additional Information

This is a fixed-term maternity cover role, with an expected duration of 9-12 months. An earlier start is preferred to allow for a full handover.

Job responsibilities

Job Role & Purpose

The Care Co-ordinator is an integral member of Aspiro Healthcares Community Support Team (CST), which provides proactive, joined up care for people with long term conditions, frailty or complex needs.

The CST works collaboratively across general practice, community services and partner organisations to promote independence, prevent avoidable admissions and improve quality of life.

The post holder acts as the central communication link for patients, carers and professionals ensuring that care plans are implemented, information shared and actions completed promptly.

This role is a temporary maternity cover post employed directly by Aspiro Healthcare.

Key Duties & Responsibilities


1. CST Case Work and Meeting Coordination

* Plan, schedule and record CST meetings, distributing agendas and documentation in advance.
* Capture accurate minutes and track progression agreed actions
* Maintain the CST database and produce caseload/performance summaries for the practice.


2. Patient Identification and Support

* Work with patients, families, and carers to develop, review, and maintain personalised care plans reflecting individual goals and preferences.
* Support patients in preparing for and following up on consultations, improving their understanding of treatment, medication, and self management.
* Identify and proactively engage patients who may benefit from care coordination, particularly those with complex needs or frequent admissions.
* Provide information, signposting, and access to self management programmes, community support, or voluntary services.
* Encourage patients to participate in decisions about their care and promote independence.


3. Care Coordination and Navigation

* Act as the link between primary, secondary, community, and social care services to ensure continuity of care.
* Track hospital admissions/discharges, ensuring timely follow up and transition of care.
* Liaise closely with GPs and practice clinicians to maintain oversight of caseloads and ensure appropriate referrals.
* Attend and contribute to multidisciplinary team meetings as required.
* Raise awareness among colleagues of patients who would benefit from shared decision making conversations or coordinated support.


4. Data Management and Reporting

* Maintain accurate and up to date records of patient contacts, interventions, and outcomes within the clinical system (e.g. SystmOne).
* Ensure care plans, key updates, and correspondence are coded correctly using established protocols.
* Produce activity data and simple performance reports to support audits, service evaluation, and quality improvement.
* Handle all information in accordance with Data Protection Legislation, confidentiality policies, and practice protocols.


5. Communication and Collaboration

* Build strong relationships across the practice and with external partners such as social prescribing link workers, health & wellbeing coaches, and community teams.
* Contribute ideas to improve workflows and patient experience, identifying opportunities to enhance coordination processes.
* Participate in internal and cross practice meetings as an active team member.
* Support induction of new staff to promote understanding of care coordination processes.


6. Professional Development

* Undertake ongoing personal and professional development, including PCI accredited care coordinator training (if not already completed).
* Participate in regular supervision and learning activities relevant to the role.
* Adhere to all mandatory training and practice policies, including safeguarding, confidentiality, equality and diversity, infection control, and health & safety standards.


7. Confidentiality

In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately. Post-holders must adhere to the following principles:

In the performance of the duties outlined in this job description, the post-holder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers.

They may also have access to information relating to the practice as a business organisation. All such information from any source is to be regarded as strictly confidential.

Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with Aspiro policies and procedures relating to confidentiality and the protection of personal and sensitive data.


8. Health & Safety

The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the Aspiro health & safety policy, the health & safety manual, and the Aspiro infection control policy and published procedures. This will include:

* Using personal security systems within the workplace according to Aspiro guidelines
* Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks;
* Making effective use of training to update knowledge and skills;
* Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards;
* Active reporting of health and safety hazards and infection hazards immediately when recognised;
* Keeping own work areas generally clean and overseeing Site Managers in the maintenance of general standards of cleanliness consistent with the scope of their role
* Undertaking periodic infection control training (minimum annually);
* Reporting potential risks identified to the Aspiro Management Team.

Person Specification


Qualifications

* Educated to GCSE level (including English & Maths) or equivalent experience
* NVQ Level 3 in Business Administration, Health & Social Care or equivalent experience
* Understanding of confidentiality and data protection requirements
* Care Co-ordinator or personalised care training


Experience

* Experience working within healthcare, social care or community services
* Experience dealing with vulnerable or complex patients
* Experience of administration, coordination or caseload management
* Experience working within a multidisciplinary environment
* Understanding of long-term conditions, frailty or complex care needs
* Experience using clinical systems such as SystmOne
* Understanding of current NHS/community care services
