Overview

This is an exciting opportunity for an enthusiastic, inspiring and reliable Community Health Care Assistant who is willing to support and deliver a range of high quality nursing and phlebotomy services to patients in their own homes who are registered with GP Practices within Chester South. Working within our integrated team, you will undertake duties involving the delivery of nursing services using a person-centred approach. This involves providing quality care to patients in their own home, or patients in long term care settings, avoiding unnecessary admission to hospital.


Responsibilities

* Provide and support delivery of nursing and phlebotomy services to patients in their own homes registered with GP Practices within Chester South.
* Deliver nursing services using a person-centred approach to promote quality care.
* Visit patients in their own environment or in long-term care settings and support to prevent unnecessary hospital admissions.
* Collaborate with an integrated team to ensure high-quality services are delivered.
* Travel to patient locations as this is an integral part of the post; access to transport for work purposes is essential.
* Demonstrate excellent communication and interpersonal skills with the ability to be adaptable, trustworthy and approachable.


Requirements / Qualifications

* Previous experience in this role would be an advantage.

About Cheshire and Wirral Partnership (CWP)
