Job overview

An exciting opportunity has arisen for an experienced and highly motivated individual to join our Adult Community Nursing Team as an Improvement Nurse.

The post holder will be responsible for working with community teams to ensure that that the correct systems are in place for the management of patients with pressure ulcers. They will use quality improvement and transformation to embed a culture of continuous improvement through collaboration and co-production with staff, patients and carers.

The successful candidate will be able to build strong relationships with internal and external partners. If you have a passion for improvement and change then this is an exiting new role which the successful candidate can support us to develop.

Main duties of the job

1. The post holder will practice within the Adult Community Nursing Team and provide direct highly skilled individualised patient care based on the principles of best practice.
2. They will be responsible for ensuring that teams are delivering a high standard of holistic nursing care in community settings including clinics and patients’ homes for patients with pressure ulcers and those who are at high risk of developing pressure
ulcers.
3. They will support other services, external partners (including carers) with expert advice and support in best practice of pressure ulcer prevention and management.
4. They will work closely with the service leads, tissue viability service, safeguarding team and improvement team using quality improvement to drive continuous change.
5. Facilitating change management to ensure the outcome of continuous clinical improvement.
6. Providing support in quality improvement methodologies, tools and techniques to support risk management and technological information where appropriate.
7. Supporting data analysis, using or developing robust data analysis tools.

Education / Qualifications

Essential criteria

8. Registered General Nurse (NMC Registered)
9. Specialist Practitioner District Nurse Qualification
10. Mentorship and Preceptorship/ Practice Assessor training
11. Evidence of ongoing specialist post registration training.

Desirable criteria

12. Leadership and Management training
13. Physical Assessment qualification

Skills/ Abilities

Essential criteria

14. The ability to act as a clinical leader, effectively prioritise own workload and the workload of others
15. Clear and effective communicator both orally and written
16. Able to use common IT packages
17. Able to develop and introduce new systems and processes through collaboration and coproduction
18. Able to demonstrate effective performance management
19. Expert clinical skills in meeting the complex needs of individuals and their families, and pressure ulcer prevention and management

Experience

Essential criteria

20. Extensive post registration experience
21. Significant experience in Community Nursing Setting
22. Working within a multi-disciplinary team
23. Experience of identifying and reporting safeguarding concerns
24. Quality Improvement and change management

Desirable criteria

25. Safeguarding Adult Reviews
26. Experience developing and running teaching sessions

Knowledge

Essential criteria

27. Knowledge/understanding of confidentiality issues and data protection standards
28. Health care within an inner city, culturally diverse community
29. Current national and local strategy
30. Understanding of Quality and Risk agenda
31. NMC code of conduct
32. Experience of managing teams
33. Experience of working with key stake holders and external Partners
