Job summary: A number of Individual Patient Activity (IPA) roles are available for IPA Adult Teams throughout Lancashire & South Cumbria. Place bases include: Blackpool, Fylde & Wyre, Morecambe, South Cumbria, Lancaster, Blackpool with Darwen, Greater Preston, Chorley & South Ribble, East Lancashire & West Lancashire. The role is family friendly and open to all allied health professionals and nurses. We are looking for candidates across the Lancashire and South Cumbria area.
The post holder will have a lead role within the IPA Adult Team concerning the commissioning and reviewing arrangements associated with adult complex client placements, including S117 aftercare, Non-CHC funded cases, and Joint funded packages of care with Local Authority. The post holder will lead case reviews (Care Programme Approach, CPA), monitor standards of care and treatment, and liaise with statutory and non-statutory agencies in the repatriation of Lancashire & South Cumbria residents. The post holder will attend Triage and Personal Health Budget (PHB) meetings to assist MDT members with funding decisions. The role involves a mix of agile working, some office-based work, and visits to people in their homes and placements.
Main duties of the job: We are looking for a motivated and experienced RMN, RGN or AHP who can work autonomously and collaboratively in a multi-agency environment to support the assessment and review process within the IPA Adult Team.
Responsibilities
 * Monitor progress on treatments
 * Inform future commissioning arrangements
 * Assist in identifying future resource implications
 * Ensure informed information is available on care and treatment to assist commissioning
 * Investigate sources and reasons for referrals and influence future referral patterns
 * Determine compliance with the ICB commissioning policies
 * Support complex MDT meetings to ensure the most effective care/treatment in the most appropriate location
 * Achieve the most effective use of the ICB's resources
 * Monitor compliance with terms of authorised referrals
 * Initial visits to potential providers and ongoing attendance at relevant review meetings
 * Liaison with client's key workers from statutory and non-statutory agencies
 * Provide and receive highly complex, sensitive information; communicate complex client condition related information to health and social care professionals/clients and other staff
 * Keep up to date with Department of Health Guidance Circulars and ensure proper implementation of policies and procedures
About us
Lancashire & South Cumbria ICB is a fully authorised ICB responsible for statutory duties and powers. The ICB aims to improve health and reduce health inequalities through clinically-led commissioning of high-quality healthcare services. The postholder will be involved in commissioning and reviewing arrangements for adult complex client placements, with good induction and training offered. The role involves working with Local Authority, Acute Trusts and providers. You will work within the Mental Capacity Act and gain experience involving Best Interests decisions and Court of Protection work. Hours are Monday–Friday, 9am–5pm with agile working possible. The ICB supports personal and professional development.

Experience and Qualifications
Experience Essential: Previous experience in a clinical role with knowledge of clinical settings and pathways; experience evaluating and measuring service performance
Knowledge Essential: Understanding of health and social care environment; ability to manage own workload and make decisions with limited information under tight timescales
Personal Qualities Essential: Adaptability, flexibility, ability to work independently, manage deadlines in changing timescales
Qualifications Essential: Degree level in a relevant subject or equivalent experience; nursing qualification or equivalent HCP (e.g., Occupational Health, Physiotherapy)
