Purpose
Trailblazer is a government-funded programme that offers intensive MSK and employment support. This service is designed to integrate an individual’s health recovery journey with their employment support into a holistic pathway.
Within our Community Health & Wellbeing division, Trailblazer is a growing part of our employment support services. We collaborate with Musculoskeletal (MSK) teams, GP practices, and other referral partners who direct participants currently unemployed or employed but facing workplace difficulties. We provide support to help them return to work, stay in work, or find new employment.
We are seeking a Health and Employment Coach to deliver effective support, guiding individuals on their journey to employment while also enhancing their health and wellbeing. The coach will work collaboratively with specialists in a community hub environment to assist participants in overcoming MSK-related barriers.
This contract is initially for 12 months with ongoing review. We are considering various options for staff recruitment, including retention incentives for new hires.
