About the Role
Are you a confident and motivated healthcare professional who works well in a multi-disciplinary team environment? Stevenage South PCN are seeking a skilled advanced practitioner to join our supportive team and deliver high-quality, safe, patient-centred care.
Based in Stevenage, Hertfordshire, this position is offered on a part-time basis for 16-22.5 hours / 2-3 days per week (exact days to be agreed with the right applicant).
This is a contract position, fixed-term until 31st March 2026 but has the potential for extension.
Main responsibilities
* Conduct thorough clinical assessments and formulate management plans for patients with minor illnesses.
* Assess, diagnose, and manage patients presenting with a variety of minor illnesses across primary and urgent care settings.
* Prescribe independently, providing efficient and effective treatment, reducing waiting times and improving patient outcomes.
* Work collaboratively with GPs, nurses, pharmacists, and other team members to deliver seamless care.
* Provide health promotion advice and support patient self-care.
* Maintain accurate clinical records and comply with all governance and regulatory standards.
About the Candidate
To be considered for this position, you will have the following;
* Registered healthcare professional (Nurse, Pharmacist, Paramedic, or other allied health professional) with a valid Independent Prescribing qualification.
* Experience and training in minor illness assessment and management.
* Strong clinical reasoning skills and sound knowledge of current prescribing guidelines (e.g., NICE).
* Excellent communication and interpersonal skills, with the ability to work well within a team and support fellow colleagues (clinical and non‑clinical)
* Commitment to ongoing professional development and high-quality patient care.
