HCA focused on Chronic Disease Management

Cranham Village Surgery, consistently voted the Number 1 GP surgery in Havering, is recruiting an experienced and motivated Health Care Assistant specialising in chronic disease reviews to join our friendly, established nursing and clinical teams.

This is a fantastic opportunity for an HCA who genuinely enjoys long-term condition work - diabetes, hypertension, CVD, CKD, COPD, asthma and frailty - and who wants to do it properly: in a well‑organised, well‑supported practice that values continuity, kindness and getting recalls right.

You will play a central role in delivering our QOF reviews, the NEL LTC Proactive Care QOF, and the Havering Network Contract LES, working alongside our practice nurses, GPs, clinical pharmacist and care coordinators. You will be supported with structured templates, clear protocols and admin support.

We are equally happy to hear from experienced LTC HCAs and from HCAs with a strong general primary‑care background who are ready to specialise.


Main duties of the job

Deliver structured chronic disease reviews in accordance with national and local guidance

* Support patients with self management -lifestyle advice and prevention strategies
* Work collaboratively within the multidisciplinary primary care team
* Maintain accurate, timely and professional clinical records + do simple tests like blood pressure, urine dip, height, weight etc…

Job responsibilities

* Deliver structured annual review appointments for patients with Type 2 diabetes, hypertension, cardiovascular disease (CHD, stroke/TIA, PAD, AF), heart failure, CKD, COPD, asthma, and other long‑term conditions.
* Complete the eight diabetes care processes (BMI, BP, HbA1c, foot risk stratification, urinary ACR, serum creatinine, smoking status, cholesterol).
* Undertake blood pressure measurement, including ambulatory and home BP monitoring set‑up and download.
* Carry out urinalysis and arrange/process urine ACR samples.
* Carry out diabetic foot risk stratification and basic foot assessment (training supported).
* Record height, weight, BMI, waist circumference, pulse and oxygen saturations.
* Calculate and record QRisk and Q‑Diabetes scores where indicated.
* Record smoking and alcohol status (including AUDIT‑C) and provide brief lifestyle advice and onward signposting.
* Complete depression screening (PHQ‑2/9), dementia screening prompts and frailty screening as part of Part A reviews.
* Identify carers, complete carer enquiry and refer to social prescribing as appropriate.
* Support delivery of NHS Health Checks, new patient checks and travel health screening as required.
* Administer B12 injections, flu and other vaccinations within Patient Group Directions / Patient Specific Directions and individual competency.
* Provide chaperoning as required.
* Support care home rounds and chronic disease work in care home residents in line with the Supplementary Care Home Service Specification.
* Use EMIS Web and Accurx accurately and efficiently, ensuring correct clinical and payment coding for QOF, PCQOF and LES reviews.


Person Specification


Experience

* Experience working as an HCA in primary care, community or a relevant clinical setting
* Experience of BP measurement on adult patients
* Experience using a clinical IT system
* Experience of structured chronic disease review work (diabetes, hypertension, CVD, CKD, COPD, asthma)
* Experience of EMIS Web and Accurx
* Experience of QOF and LES coding and recall
* Experience working in or with care homes


Qualifications

* NVQ Level 3 in Health and Social Care (or equivalent) or Care Certificate plus relevant primary care HCA experience
* BP measurement training
* Up‑to‑date mandatory training (Safeguarding Adults/Children, BLS, IPC, Information Governance) – we can support with this
* Recognised HCA chronic disease / long‑term conditions training (e.g. diabetes, COPD, CVD, asthma modules)
* ECG training
* Immunisation training
