Come and work with us in the Swadlincote Integrated Community Team where we have a well-established MDT and can give you the opportunity to see patients with a wide range of multi‑pathologies.
Team Composition
The therapy team is comprised of Physiotherapists, Occupational Therapists and Support Workers who work closely with the wider multidisciplinary team to provide rehabilitation to patients in their own homes.
Role Overview
This post is an excellent opportunity to develop your skills and knowledge working within supportive environments with access to a preceptorship programme. Our Integrated Community Services Teams are enthusiastic, with a breadth of experience and a shared goal to offer the highest standard of care to our patients.
Responsibilities
* Carry your own clinical caseload
* Attend regular therapy meetings
* Undertake individual and joint therapy assessments and treatment sessions with membership of the MDT where appropriate
* Deliver rehabilitation to patients in their homes, focusing on improving health and well‑being, working toward patient‑centred goals and preventing hospital admission or facilitating discharge home
Development Opportunities
You will be able to develop your knowledge and experience around Falls and Frailty and Triage.
Qualifications & Skills
* Good communication skills
* Ability to plan and organise
* Evidence‑based approach to care
* Previous post‑qualification experience desirable but not essential
