East Lothian Health and Social Care Partnership is seeking two experienced and motivated Social Workers to join our newly established Prevention and Early Intervention multi-disciplinary team.

This innovative team works collaboratively with NHS colleagues and partners from Community First, hospital flow and discharge services, reablement, social work, and care-at-home teams. Our core focus is to:

* Prevent unnecessary hospital admissions

* Support timely and safe discharges
\n
* Reduce the impact of unscheduled care
\n
* Promote recovery at home
\n
\n
As a Social Worker in this team, you will:

* Adopt an “In Reach” approach within Edinburgh hospitals

* Carry out assessments for individuals in the community
\n
* Deliver responsive, person-centred support to enable independence and recovery
