Location: Flintshire | Hours: 37.5 per week | Community-Based Role

What You'll Be Doing
As a Field Support Supervisor, you'll play a key leadership role in delivering high-quality, person-centred care and support. You'll:
* Develop and maintain personalised support plans
* Lead, motivate and supervise support teams
* Conduct assessments and audits to ensure outstanding care
* Support with medication management and compliance
* Attend multi-disciplinary meetings and contribute to care planning
* Take part in the on-call rota to ensure round-the-clock support
* Be a visible and inspiring presence in our services, building strong relationships with the people we support and your team
Who We're Looking For
We're looking for someone with passion, professionalism, and the experience to lead with empathy. You should have:
* Previous experience in a care management role (CIW preferred)
* A Level 3 Health & Social Care qualification. (If not held be willing to enroll)
* Experience supporting individuals with learning disabilities, physical disabilities, autism, ABI, mental health needs, trauma backgrounds, and behaviours that challenge.
* Strong communication, organisational and leadership skills
* A commitment to quality and person-centred care
* Willingness to work flexibly across multiple sites (including weekends, evenings, and sleep-ins)
