You will provide in-reach to secure wards and follow up to 30 in-patients prior to discharge but you will not be Responsible Clinician (RC) for these patients. You will offer clinical leadership to the team and act as Responsible Clinician / Clinical Supervisor for the service users in your caseload, who will also have an assigned social supervisor and CPN.
