Job Type: Full-Time | Locum Contract
Schedule: Monday - Friday | up to 34hrs per week
Location: Slough, SL2 | Onsite
Pay:Dependent on experience

What You’ll Do:
As a Clinical Pharmacist (Independent Prescriber), you’ll work directly with GP practices and PCNs to:
1. Optimise medication use and outcomes in long-term conditions (asthma, hypertension, diabetes)
2. Conduct structured medication reviews and manage repeat prescriptions
3. Provide expert advice on acute requests, discharge summaries, and clinical letters
4. Support QOF and IIF performance
5. Empower patients through education and guidance
Essential Criteria:
6. GPhC-registered Pharmacist with Independent Prescribing qualification
7. Clinical experience managing asthma, hypertension or diabetes
