Overview
Lincolnshire Community Health Services are seeking a full‑time District Nurse / Case Manager to join the Lincoln North Community Nursing Team at Ravendale Health Centre.
Qualifications
* Hold the Community Specialist Practitioner qualification.
* Experience managing a caseload of patients and leading a team in delivering excellent patient care.
* Alternatively, a Case Manager without the DNSPQ may be considered for the right candidate with a minimum of 2 years experience within a Community Nursing Team and willingness to undertake the DNSPQ course.
Responsibilities
* Hold a caseload of patients with varying health needs, coordinating care delivery.
* Display extensive communication, interpersonal and clinical skills.
* Lead the community nursing team through challenging times and service transformation.
* Role model excellent behaviours in line with trust values to improve quality of care and a positive patient experience.
