Join our NHS programme, High Intensity Use (HIU) Programme as an Outreach Caseworker, supporting vulnerable individuals across East Riding of Yorkshire and North Lincolnshire. This initiative transforms care for frequent users of urgent and emergency services. You’ll support individuals who frequently access services—empowering them to take control, avoid crisis, and reconnect with their communities.
We welcome applications for both full-time and part-time. Depending on your location, you may cover East Riding of Yorkshire and North Lincolnshire, or focus on one area, with flexibility to cover both based on service demand.
Responsibilities
* Supporting individuals with complex needs to access the right help and avoid crisis.
* Identifying social factors driving high use of emergency services.
* Building trust with people facing challenges like trauma, addiction, and mental health issues.
* Reducing stigma and shifting behaviours through community engagement.
* Working closely with partners like A&E, GPs, housing teams, and mental health services.
Qualifications and experience
* Experience supporting vulnerable people with complex needs, e.g. substance abuse, mental health conditions.
* Strong communication skills and the ability to build trust.
* Confidence in delivering casework, person-centred planning and risk assessment.
* Quick thinking and effective negotiation in fast-paced settings.
* Accurate record-keeping and attention to detail.
