  • Assess medicines related needs, support adherence and non-compliance, and facilitate implementation of safe handling and administration of medicines.
  • Demonstrate collaborative working with the wider health and social care multidisciplinary team to reduce medication related risk to patients in their own homes.
  • Participate in the delivery of medicines optimisation initiatives.
  • Participate in audits within the Trust.
  • Develop and provide a clinical pharmacy technician role to patients across the Westminster and K&C areas.
  • Take referrals and triage them from a wide range of health and social care professionals to develop pharmaceutical support packages for patients with complex medication needs in order to ensure they are competent in the use of their medicines.
