Overview
Health Promotion Outreach Worker role at NHS Homerton Healthcare NHS Foundation Trust. The role involves conducting health promotion activities for the NHS bowel cancer screening programme across City & Hackney, Newham, Tower Hamlets and Waltham Forest. The candidate will raise awareness, identify and reduce barriers to improve uptake of bowel cancer screening across participating London Integrated Care Boards (ICBs).
The post holder will work closely with the Health Improvement Specialist to develop and implement a coordinated programme with Primary Care staff and Community Organisations. The aim is to improve uptake and coverage to a minimum national standard. A key duty is to raise awareness of the bowel screening programme with Local GPs, public health teams, community groups, councils and the general population in the localities served by the Bowel Cancer Screening Centre (BCSC).
You should be approachable, flexible and committed, able to develop creative solutions, and an exceptional communicator capable of working positively and collaboratively. A good understanding of the North East London area is desirable.
Main duties
* Deliver a programme of evidence-based health promotion activities with Primary Care and Community Groups to reduce barriers to bowel cancer screening across the CCGs
* Conduct health promotion activities in accordance with the BCSC Health Promotion Strategy
* Assist in needs assessments to inform professionals on community requirements regarding bowel cancer and screening
* Integrate bowel cancer prevention within broader health programmes in primary care and community settings
* Create and provide appropriate health information mindful of language and literacy needs, ensuring availability when required
* Work with individuals using behaviour change/motivational interviewing techniques
* Contribute to the evidence base for increasing access to bowel cancer screening
* Develop health champions to increase screening acceptability among eligible populations
* Support administrative tasks and ensure outreach activities are recorded on the BCSC Health Promotion Monitoring system to inform service review and development
* Ensure health inequalities, inequity and vulnerable individuals and communities are understood and addressed in health promotion interventions
* Maximise the use of professional supervision to ensure targets are met

Essential
* Education/Qualifications: Certificate/Diploma in health/community development or relevant experience
* Skills/Abilities: Excellent communication with stakeholders; knowledge of health promotion methods; ability to tailor health information to language and literacy needs; strong interpersonal and team skills; good administrative and IT abilities; confident public speaking with audiovisual aids; problem solving; empathy
* Experience: At least 2 years in Health Promotion or a related field; experience of development, implementation and evaluation of health promotion programmes with evidence base across partnerships
* Knowledge: Knowledge of the national Bowel Cancer Screening Programme objectives; understanding of statutory, voluntary and community sectors; understanding of health determinants and Public Health; experience working with targeted populations; awareness of equality opportunities
