Job Summary
The postholder will lead digital transformation and service improvement across the Primary Care Network (PCN). Using a data‑driven approach, they will analyse service models, identify challenges, prioritise improvements and monitor impact.
Key Responsibilities
* Drive digital transformation across ten practices, improving access, efficiency and patient outcomes.
* Use data tools (RAIDR, QOF) to identify health inequalities, streamline pathways and support performance improvement.
* Promote NHS App uptake, standardise communication, deliver digital training and explore AI solutions.
* Support data‑informed decision‑making, establish effective data‑sharing mechanisms and track progress using analytics.
* Apply project‑portfolio management (PPM) techniques to plan, deliver and evaluate transformation initiatives.
* Analyse population health data to address inequities and guide the PCN improvement plan.
* Report regularly to PCN leadership on outcomes, challenges and opportunities for continuous improvement.
Qualifications and Experience
* GCSE Maths & English or equivalent.
* Experience in general practice or primary care.
* Project management qualification (e.g. PRINCE2) or equivalent experience.
* Experience with EMIS, RAIDR, QOF and Microsoft Office (Word, Outlook, Excel).
* Strong data handling and reporting skills.
* Excellent interpersonal and communication skills.
Technical Skills
* Project management, data management principles and leveraging healthcare data for decision‑making.
* Strategic thinker capable of developing long‑term digital strategies.
* Excellent written and verbal communication.
Desirable Knowledge
* GDPR / Data Protection, Health & Safety, Confidentiality, Safeguarding.
* Awareness of national and local digital priorities.
