Monday- Friday Shift patterns: 08:00-16:00, 08:30-16:30, 09:00-17:00

Permanent, full time

The delivery teams within the Care Assessment area of Xyla’s Healthcare Services division consist of dedicated operational and clinical professionals who provide a successful range of assessment services. Their work is driven by a shared commitment to improving people’s lives.

The Contract Coordinator plays a vital role in supporting the division by managing the coordination of NHS Continuing Health Care packages, attending and minuting appeal meetings, and Organising reviews and assessment meetings. This involves liaising with a wide range of stakeholders, including internal teams, external professionals specifically within the Continuing Health Care assessment service line.

This is a fast-paced role with a strong focus on meeting operational KPIs in accordance with service agreements, whilst ensuring empathy to patients and families often at a great time of stress/uncertainty.

Every day you will…

* Coordinate reviews and/or assessments, as well as multi-disciplinary team meetings to set KPIs and ensure efficient and effective project delivery.
* Liaise with care homes, care providers and GPs to gather necessary clinical documentation for assessment meetings.
* Act as a central point of contact for the department, liaising with our clinical teams, ICBs, social workers, solicitors/advocates and patients and their families.
* Share updates and acting as the link between services and patients/families.
* Establish and maintain relationships with a range of stakeholders.
* Identify ways to improve processes and service delivery.
