To deliver a high-quality care co-ordination service to patients and healthcare professionals.
Accurately record all relevant data for each contact on a range of clinical systems and direct appropriately. Complete all necessary checks, such as NHS Spine, when demographics are incomplete, to ensure the most up-to-date records are available.
Undertake the booking of appointments across various services in line with agreed timescales. Provide a first-class reception service, ensuring a professional meet and greet, efficient registration, and onward referral to the relevant area/service.
Liaise with the Senior Care Co-ordinator regarding any operational issues affecting patient care. Follow the failed encounter procedure for all patients who did not attend (DNA) an appointment. Liaise with the multidisciplinary team to ensure the smooth running of the service. Liaise with GP practices regarding patient referrals.
Facilitate the transfer of electronic data both internally and externally, ensuring that information governance rules are followed at all times.
Have a full understanding of the clerical backup system to facilitate smooth transactions from computer-based to paper-based systems. Fully comply with the Complaints Policy, ensuring all informal and formal complaints and incidents are documented and the process is explained to patients.
Undertake administration duties within the service and ensure regular attendance at operational meetings. Complete all mandatory training within the specified time frames.
Perform any tasks as requested within the role's boundaries. Ensure that all GTD Healthcare policies and procedures underpin decision-making. Other duties may include travel to other sites within the organization.
