Overview

Permanent, full-time Brain Injury Case Manager role in Cheadle, Stockport for TieTalent. Monday–Friday, 9:00–17:00 (part-time considered). This role is office and community-based, with a caseload primarily in the Greater Manchester, Lancashire & Leeds areas. We are looking for a Nurse, OT, Physiotherapist, Social Worker, or registered health or social care professional to join our case management team. You will work with paediatric and adult clients who have sustained neurological or complex orthopaedic injuries. An induction programme, regular supervision and ongoing mentorship are provided.


Responsibilities

* Assess immediate and ongoing client needs, set goals, arrange reviews and coordinate services.
* Adopt a creative, proactive approach; maintain risk awareness and demonstrate a dynamic, positive attitude.
* Communicate effectively with clients and professionals; maintain professional boundaries and confidentiality; advocate for clients.
* Work as part of a strong, supportive team; this role is primarily office and community-based.
* Deliver occasional evening training and networking opportunities as required.


Application process

Salary and benefits

Salary: £42,000 – £48,000 dependent upon relevant experience. Benefits include Health Cash Plan, Employee Assistance Programme, ongoing training, regular supervision, 25 days annual leave (plus bank holidays), 4 weeks full sick pay, client mileage and expenses.

* A professional qualification and current registration.
* 2 years post-registration experience.
* Ability to dynamically risk assess and creatively problem-solve.
* Commitment to client safeguarding, dignity and respect.
* Ability to maintain professional boundaries and confidentiality.
* Ability to advocate for clients.
* Ability to identify and monitor client goals.
* Punctuality and excellent time management.
* Full driving licence with access to a reliable vehicle insured for business use.
* Ability to work well within a team and have a positive and caring attitude.
* Ability to work from home and ensure a secure and confidential home-working environment.
* Flexible and creative thinking.
* Ability to incorporate client and company values into your practice.
* Ability to attend occasional evening training and networking opportunities.
* Excellent communication skills and standards, both verbal and written.
* Excellent IT skills, particularly proficiency in Excel, Microsoft Word and Outlook.
* Ability to maintain a professional image.
* Patience and a non-judgmental approach.
* Excellent sense of humour.


Preferable criteria

* Experience in the social care and rehabilitation settings.
* Case management experience.
* Knowledge/experience of brain and spinal injury.
* Experience of complex and challenging behaviour management.
* Experience of working within the medico-legal arena.
