Job Title: Adult Social Worker - Hospital Discharge Team Location: South Shields, Tyne and Wear, United Kingdom Salary: £24 per hour We are seeking a dedicated and compassionate Adult Social Worker to join our Hospital Discharge Team in South Shields. As an integral part of our team, you will play a crucial role in facilitating the safe and timely discharge of adult patients from hospital to their homes or appropriate care settings. Responsibilities: Conduct comprehensive assessments of patient's social care needs, taking into account their physical, emotional, and psychological well-being. Develop and implement personalised care plans in collaboration with patients, their families, and multidisciplinary health-care teams. Coordinate discharge planning processes, ensuring seamless transitions for patients from hospital to their homes or alternative care settings. Provide support and guidance to patients and their families, signposting to relevant community resources and services. Advocate on behalf of patients to ensure their rights and preferences are respected throughout the discharge process Collaborate closely with health-care professionals, including doctors, nurses, occupational therapists, and physiotherapists, to optimise patient outcomes Maintain accurate and up-to-date records of assessments, care plans, and interventions, adhering to relevant legal and professional standards.A strong Candidate will have: Previous experience working in a similar role within a health-care or social care setting Knowledge of relevant legislation, policies, and procedures related to adult social care and safeguarding. Strong assessment and decision-making abilities, with a person-centered approach to care. Excellent communication and interpersonal skills, with the ability to engage effectively with patients, families, and multidisciplinary teams.
