We have an exciting opportunity for an experienced nurse to join ouraward-winning stoma care team on a secondment to cover maternity leave. The successful candidate will provide specialist support to patients requiring stoma formation, management and ongoing care.
The role involves supporting patients pre –operatively, through surgery, recovery, rehabilitation and long term follow up.You will assess patients holistically, providing expert clinical advice, education, and psychological support to help individuals adapt to living with a stoma.

The Stoma Care Nursing Team places a strong focus on collaborative working across surgical teams, ward staff, community services and the wider multidisciplinary team to maintain excellent standards of care. In addition to direct patient care, the role would involve ward based education, contributing to service improvement initiatives.
You will be supported to gain skills in work autonomously and make safe, effective clinical decisions. You will work closely with the colorectal surgeons, ward teams and community partners. We require you to be band 5 or above, with relevant colorectal, surgical experience.
Main duties of the job
1. Deliver pre- operative counselling and stoma site marking (with support as appropriate)
2. Provide post-operative stoma education and practical support to patients and their carers/family members if required.
3. Assess, manage and advise on stoma related complications.
4. Contribute to nurse-led follow up clinics and telephone support.
5. Support discharge planning and continuity of care with community services.
6. Deliver education and support to ward staff and junior colleagues.
7. Contribute to audit, service development and quality improvement initiatives

Detailed job description and main responsibilities
The role encompasses the following:
8. To work as an autonomous Specialist Nurse within the Stoma Care Service.
9. To have a designated caseload and coordinate and plan care as keyworker for patients requiring stoma formation and ongoing specialist stoma support.
10. To provide expert advice related to stoma management, treatment pathways and rehabilitation, with a focus on improving patient care and contributing to service development.
11. To assess patients holistically, plan, implement and evaluate evidence-based care for patients requiring temporary or permanent stomas.
12. To provide specialist advice to patients, families and carers, supporting physical, psychosocial and emotional adjustment to living with a stoma.
13. To carry out specialist nursing procedures in relation to pre-operative counselling, stoma siting, post-operative education and management of stoma-related complications.
14. To contribute to optimisation of patient outcomes through proactive assessment, early intervention and ongoing specialist review.
15. To work collaboratively with colorectal consultants, ward teams and the wider multidisciplinary team to ensure cohesive management of patients across the pathway.
16. To act as a keyworker for designated patients as appropriate across the patient pathway.
17. To contribute to the development of the workforce through developing and delivering education and training to nursing and multidisciplinary staff.
18. To supervise delivery of a range of clinical and core nursing skills and interventions to meet physiological and psychosocial needs.
19. To contribute to nurse-led clinics, telephone support and follow-up services within the Stoma Care Service.
20. To assess and manage complex stoma-related complications including skin integrity issues, leakage, prolapse, retraction and high-output stomas, escalating concerns where appropriate.
21. To support discharge planning and continuity of care through effective liaison with community services and external agencies.
22. To lead and contribute to service development initiatives within the team.
23. To participate in research activity, audit and clinical governance within own specialist area.
24. To contribute to the review and development of clinical pathways, guidelines and patient information resources.
25. To work autonomously, demonstrating safe clinical decision-making and accountability within professional scope of practice.
26. To maintainaccurate documentation and ensure care is delivered in accordance with Trust policies, professional standards and governance requirements.
27. To promote high standards of specialist stoma care through evidence-based practice, innovation and continuous quality improvement.

Knowledge & Experience
Essential criteria
28. Senior nursing experience in appropriate clinical area of colorectal patients, with evidence of leading the day to day running of the charge
29. Knowledge of Clinical Governance and Safeguarding and the relevance of these to practice
30. Evidence of clinical competence in area of speciality eg colorectal surgery and stoma care
31. Knowledge of local/national Health and Social Care policy relating to colorectal cancer
32. Knowledge and experience in caring for patients along the colorectal cancer pathways

33. Experience of managing within a clinical area
