Practice Nurse (Diabetes Focus – Part Time)

Location: Martlesham Heath Surgery, 23 The Square, Martlesham Heath, Ipswich, IP5 3SL.

Job Summary

You will be a passionate and motivated Practice Nurse with a focus on diabetes care and long‑term condition management. In this role you will deliver high‑quality, evidence‑based care across chronic disease management, health promotion, screening and preventative services, while working within an experienced multidisciplinary team.

Main Duties of the Role

* Deliver high‑quality, evidence‑based nursing care
* Undertake diabetes reviews, monitoring and patient education
* Support the management of long‑term conditions such as asthma, COPD and hypertension
* Provide routine nursing services: immunisations, wound care, cervical cytology (if trained), health checks and screening
* Promote health and wellbeing through lifestyle advice
* Maintain accurate and contemporaneous clinical records
* Work collaboratively with GPs and the wider multidisciplinary team


Job Responsibilities


Clinical Care

* Provide high‑quality, evidence‑based nursing care in line with NICE guidelines
* Deliver routine nursing services including:
o Wound care and dressings
o Suture and staple removal
o Cervical cytology
o Immunisations (routine, childhood and travel)
o ECGs and spirometry
* Undertake new patient health checks and routine screening


Chronic Disease Management

* Support the management of patients with long‑term conditions, including diabetes (essential), asthma, COPD and hypertension
* Carry out reviews, monitoring and patient education
* Support patients to self‑manage their conditions


Health Promotion & Prevention

* Deliver opportunistic health promotion and lifestyle advice
* Support national and local health campaigns (e.g., vaccinations, NHS health checks)
* Promote patient wellbeing and disease prevention


Infection Prevention & Control

* Work in accordance with infection prevention and control policies
* Support IPC audits and compliance activities as required


Medicines Management

* Administer medications in accordance with PGDs and scope of practice
* Provide advice to patients on medication use, side effects and compliance


Patient Management & Administration

* Request and process pathology tests as appropriate
* Maintain accurate, clear and contemporaneous clinical records
* Refer patients to GPs or other health professionals when appropriate


Team Working

* Work collaboratively with GPs, nursing colleagues and the wider multidisciplinary team
* Support colleagues and contribute to a positive team environment
* Participate in practice meetings and training


Person Specification


Qualifications

* Registered Nurse (NMC)
* Diabetes qualification or relevant training/experience
* Experience in chronic disease management
* Immunisation and vaccination training
* Cervical cytology training (desirable)


Experience

* Experience working in primary care
* Experience managing long‑term conditions, particularly diabetes
* Experience working autonomously within scope of practice
* Experience with audit or quality improvement (desirable)
* Experience with elderly or housebound patients (desirable)


Clinical Skills & Knowledge

* Chronic disease management (including diabetes)
* Wound care and dressings
* Immunisations and vaccinations
* Cervical cytology
* Spirometry and ECGs
* Pathology testing and results handling
* Knowledge of evidence‑based practice and NICE guidelines
* Understanding of PGDs


Personal Skills

* Strong communication and interpersonal skills
* Good time management and organisational skills
* Ability to work independently and as part of a team
* Professional, patient‑focused approach


Additional Information

* Subject to DBS check and referencing
* Must hold current UK professional registration
