We are looking for caring and compassionate colleagues to provide purposeful, outcome focussed support to young people living at Ty Parc, our new supported accommodation project in Abergavenny. This is a chance to be part of new Taff service from the beginning. Every young person deserves the chance to take their next steps with confidence, and you could be one of the people who helps make that happen. You will support, enable, and assist five care experienced young people aged 16+ to make positive and sustainable changes to their lives.

No two days will be the same. You’ll be delivering support that is shaped by real lives, experiences and progress, helping young people build trust, confidence and stability at their own pace. This is a meaningful challenge, and you’ll be supported to do it well. At Taff we work in a psychologically informed way and aim to build resilience in trauma‑affected young people, which can bring up many difficult emotions, both for them and for the colleagues who walk alongside them.

What you’ll do
You’ll be a key part of the day‑to‑day delivery of Ty Parc, supporting care experienced young people aged 16+ as they move from residential care toward independence. You’ll hold your own caseload, leading on assessment, support planning and risk management while getting to know the young people you support, understanding what matters to them and helping them plan their next steps. You’ll also line‑manage a small team of support assistants, helping them feel confident, supported and able to work in a psychologically and trauma‑informed way. You’ll model this approach in your own work and with the colleagues you supervise and take part in an on‑call rota to support the 24/7 running of the project.
What you’ll bring to the role
* Taking a psychologically and trauma informed approach to your delivery of support in a supported accommodation setting
* Creating tailored support plans using SMART goals, delivering support and supporting residents to achieve successful move‑on into independent accommodation
* Receiving referrals, assessing risk and communicating relevant risk information clearly and appropriately
* Keeping detailed and timely notes on resident interactions and making sure that important information is shared appropriately with the team
* Liaising with partner agencies to share relevant updates, coordinate support and help keen residents safe
* Covering colleague caseloads where necessary to support continuity of service.
* Taking responsibility and ensuring the safety of residents who are in crisis responding to incidents and managing the safety of the project for residents and colleagues during those incidents
* Delivering activities and workshops for residents to support them to move on to independent living and to improve their wellbeing and support them to thrive
* Line managing support assistants and delivering regular, high‑quality supervision
* Holding and managing sensitive and confidential information appropriately
* Following Safeguarding and child protection procedures, escalating concerns appropriately
* Attending multi‑disciplinary meetings with external agencies to coordinate support, safeguard service users and share best practice
* Maintaining the safety of residents, colleagues and visitors in the building following health and safety guidelines and visitors' policy
* Taking part in an on‑call rota to respond to the needs of the project out of hours
