District Nursing Lead - Advanced District Nurse Specialist
We are delighted to offer a career development opportunity within the District Nursing Service for District Nurse Specialists to progress to a senior, advanced clinical and leadership role. Applicants who are approaching the end of their District Nurse Specialist Practitioner Qualification will be considered.
Responsibilities
* Develop and deliver the preventative care agenda.
* Provide clinical leadership for the local development of the national ageing well anticipatory care model in Oxfordshire, working closely with senior leaders.
* Build relationships and integrated working with multi‑professional teams to deliver the model in local populations.
* Ensure that planned care within District Nursing teams aligns with quality standards and evidence‑based pathways; ensure patients on the caseload have the best outcomes and that the caseload is effectively managed.
* Lead the District Nurse teams in the management of patients with more complex and/or advanced needs.
* Work in an integrated way to provide leadership and support for the locality DN teams with the District Nurse Lead and locality District Nursing Locality Lead post holders.
