Reablement Support Worker
Hours - 25
Shift pattern - shift based, working 5 days out of 7, totalling 25 hours
Location - Bradley Resource Centre (Bradley reablement residential setting)
City of Wolverhampton Council is recruiting a Reablement Support Worker to work with vulnerable adults who have suffered an accident or short-term illness. You will teach them skills to enable them to perform everyday tasks and help them regain confidence and independence. This is a rewarding role where you can gain valuable experience while helping to change someone’s life.
Contract - part time, permanent; Work Style - working from a council location (Bradley Resource Centre)

Responsibilities
* Undertake through individual care plans, personal, practical and rehabilitative duties to maximise the individual’s independence and well-being.
* Carry out therapeutic activity and skills practice in accordance with the agreed plan to improve and maintain mobility, daily living skills, transfer and building confidence.
* Encourage and support people to maintain optimum independence, including decision making, coping strategies, maintaining social contacts and activities, and managing a safe home environment.
* Accurately communicate verbally, in writing and using IT systems according to the demands of the situation to ensure safe and effective service delivery and care plan reviews.
* Observe, record and report any changes in individual conditions and circumstances and ensure they are reported to senior staff promptly, considering severity, urgency, policies, and responsibilities to safeguard health and safety.
* Research information about community resources, benefits advice, local services, cultural and religious support groups; signpost customers to appropriate provision.
* Be familiar with Equalities best practice and implement this in all working practices to ensure fair and equitable service provision.
* Maintain and comply with local policies and standards, including training and appraisal targets to continuously improve personal and service standards.
Skills & Experience
* NVQ level 2 in caring or equivalent
* Trained in the administration of medication procedures
* Trained in Moving & Handling
* Good communication skills with individuals, carers, relatives and professional colleagues
* Ability to use own initiative when reporting concerns and acting on them
* On-the-job 12-week induction with mandatory training requirements
* DBS check required prior to commencement
