Position Overview
Surrey and Borders Partnership NHS Foundation Trust seeks a dedicated Professional to support the delivery of the Trust’s Patient Safety Incident Response Plan by contributing to high‑quality patient safety incident reviews and ensuring proportionate responses to embed learning across the organization.

Key Responsibilities
* Support the delivery of the Trust’s Patient Safety Incident Response Plan (PSIRP) by undertaking and contributing to high‑quality patient safety incident reviews.
* Ensure proportionate responses are taken to identify and embed learning from patient safety incidents across the organization.
* Day‑to‑day management of patient safety incident reviews as allocated by the Head of the Safety Team.
* Undertake reviews in line with a systems learning approach, involving patients, carers/families, Trust staff and third parties where appropriate.
* Collaborate with internal and external stakeholders to review, analyse and collate evidence and findings using the agreed PSIRP methods.
* Identify systems learning from patient safety incidents to guide sustainable risk reduction and drive service improvement.
* Embed Just and Learning Culture principles and human factors concepts throughout the review process.
* Analyse and interpret complex and sensitive clinical and safety situations, using advanced communication and coaching skills to facilitate learning and improvement.
* Contribute to decision‑making processes that support patient safety, quality improvement and organisational learning.
