Job Summary
To respond quickly to all referrals for patients who have fallen or are at risk of falling, register and triage the referral in accordance with the KPIs. The role involves visiting patients at home, completing a comprehensive falls assessment, identifying necessary equipment and support, facilitating falls strength and balance classes, coordinating with various services, and maintaining accurate records.
Key Responsibilities
* Respond quickly to referral referrals and triage them per KPI requirements.
* Visit patients in their own home or residence to conduct evidence‑based falls assessments.
* Identify and order equipment, additional care or support to maintain patient safety at home.
* Facilitate and support the Luton Falls Team in delivering falls strength and balance classes and group programmes.
* Analyse facts, make judgments within competency level, and adapt assessments and recommendations as necessary.
* Make recommendations to patients and carers that reduce the risk of further falls.
* Refer to other services to support independence and reduce falls risks.
* Follow up to ensure equipment has been delivered and care is in place.
* Maintain and update patient records using an electronic system and collaborate with the Falls Coordinator to collate and analyse data.
* Plan and prioritise workload; work within the Falls Team to deliver comprehensive intervention services.
* Lead delivery of falls strength and balance classes across Luton, travelling locally between venues and patients' homes.
* Collaborate with Luton integrated community services, ambulance service and other health, social care and voluntary organisations.
* Support day‑to‑day duties such as triaging referrals, answering system queries and contacting patients and nominated 1 for any queries.
Qualifications & Experience
* Experience of working in the community.
* Time management, ability to organise and prioritise work.
* Experience facilitating exercise classes.
* Experience in lone working.
* Car ownership and UK driver’s licence.
* IT skills using Microsoft Word, Excel and PowerPoint.
* NVQ level 3 or equivalent (e.g. BSc, MSc, Diploma) entry level qualification into the Physiotherapy profession.
* Completion of application form legibly and clearly, training about falls or related topics.
