Band 6 Assessment Nurse – Decisions Unit

The closing date is 14 May 2026.

We are offering a Band 6 Registered Mental Health nursing role within the Decisions Unit at the Sheffield Health Partnership University NHS Foundation Trust. The role involves assessing patients experiencing mental health crises who present in A&E, the Health Based Place of Safety, and the wider crisis services network. The assessments support safe and effective crisis care and facilitate appropriate placement or discharge.


Main duties

* Provide assessment for people in mental health crisis who present in A&E Sheffield Northern General Hospital or to Sheffield's wider network of crisis services.
* Deliver effective, safe, high-quality crisis care within the DU / Crisis Hub, including assessment, planning, evaluation and management of crisis care, clinical need and risk.
* Work closely with the Psychiatric Liaison Service and A&E streaming nurse, acute mental health inpatient wards and varying community services to support and maintain patient flow.
* Provide clinical leadership and day‑to‑day coordination of clinical practice to support delivery of care within the DU.
* Develop strong links and protocols with other services and support transitions between services.
* Support ongoing management and development of the DU and wider crisis hub services.
* Work as an effective member of the multi-disciplinary team, maintaining high standards of patient-focused care.
* Develop team working and maintain consistent standards of care delivery throughout crisis hub services.
* Work flexibly to cover shifts covering a twenty-four-hour period to meet service need.


Job responsibilities

* Value and respond to the individual rights, needs and opinions of service users.
* Provide mental health assessment in A&E, DU and Health Based Place of Safety.
* Support patients with medication in line with service and trust guidance.
* Plan, implement and evaluate nursing care programmes to agreed standards.
* Conduct mental and physical health assessments.
* Monitor standard of care delivery by junior staff and educate colleagues.
* Maintain a client-centered approach in service delivery to older people.
* Provide nursing care for patients and support to services across the trust when needed.


General requirements

The post holder is expected to be familiar with relevant policies and legislation, including NMC guidelines and trust policies. They will develop a personal development plan through supervision and PDR.


Clinical responsibilities

* Work across a variety of community settings as part of a multi-disciplinary team.
* Conduct nursing assessment and identify mental health needs.
* Formulate, implement and evaluate nursing care plans reflecting client/carer needs.
* Manage own caseload and review with supervisor.
* Administer depot injections and monitor medication effects.Maintain accurate care records according to professional and trust standards.
* Coordinate according to the Care Programme Approach.
* Deliver therapeutic interventions with clients and carers in line with evidence-based practice.
* Respond to complex mental and physical health crises.
* Escort clients to other health and social care settings when appropriate.
* Offer support and advice to families and carers and advocate for clients.
* Identify and manage risks.
* Provide direct supervision to other staff and participate in PDR.
* Liaise with other professionals and bodies in the care of older people with mental health needs.
* Act as practice supervisor and assessor for nursing students and preceptor for nurses.
* Receive clinical supervision regularly.
* Participate in a learning environment and promote training opportunities.
* Share skills with colleagues and support development.
* Take responsibility for personal development, appraisal and clinical supervision.
* Act as a positive role model.
* Add education and health promotion to clients and carers.


Qualifications

* Registered Mental Health Nursing qualification (RMN).


Knowledge and skills

* Good risk assessment and risk management skills.
* Understanding of mental health needs of older people.
* Knowledge of supervision principles.
* Knowledge of MHA/MCA and relevant legislation.
* Skills in care planning, goal setting and discharge planning.
* Knowledge of medication and its management.
* Knowledge of physical health management.
* Interpersonal skills in motivating self and others.


Experience

* Experience of liaison with other agencies.
* Experience of treatment modalities and interventions.
* Experience of assessment and care planning.
* Relevant post-qualifying experience in mental health nursing.
* Experience of multi-disciplinary team working.
* Experience of clinical supervision.
* Experience of working in the community.
* Experience of using an appraisal system.
* Experience of quality improvement and service development.
