Job Summary
Band 5 Rotational Physiotherapist to join the East Coast Community Healthcare Physiotherapy Rotation, covering specialist services within the community.
Services include:
* Primary Care Homes – specialist physiotherapy for complex community care needs.
* Integrated Community and Neurology – support to patients with long‑term neurological conditions.
* Musculoskeletal (MSK) Team – assessment and treatment of musculoskeletal conditions.
* Intermediate Care Team – care for highly complex patients in ward‑based and community settings, emphasis on reablement.
Clinical Responsibilities
* Professional and legal responsibility for all aspects of own work; manage a designated caseload.
* Interpret and analyse clinical facts to form accurate diagnoses and develop comprehensive management strategies.
* Conduct comprehensive patient assessments and formulate individualized treatment plans.
* Demonstrate precise dexterity, coordination and palpatory skills for assessment and manual therapy.
* Provide specialist advice, teaching and instruction to relatives, carers and other professionals.
* Adhere to equipment safety policies and supervise less senior team members as appropriate.
* Document accurately and remain compliant with legal and departmental requirements.
* Communicate effectively with patients, carers and multidisciplinary team to maximise rehabilitation outcomes.
* Obtain valid informed consent and work within the legal framework for patients lacking capacity.
Professional Responsibilities
* Maintain CPD and incorporate new developments into practice.
* Participate actively in in‑service training and external courses.
* Maintain comprehensive documentation and report to appropriate disciplines.
* Engage in staff appraisal schemes and personal development plans.
* Lead or participate in evidence‑based practice projects, audits and outcome measurement.
* Demonstrate sound understanding of clinical governance and risk management.
Organisational Responsibilities
* Manage a designated area of work, plan and organise patient care efficiently.
* Prioritise tasks to align with service demands.
* Collect and analyse service data and statistics.
* Implement health and safety policies and report incidents promptly.
* Ensure service policies and developments are implemented and reviewed.
* Undertake duties delegated by senior clinicians or professional leads.

Person Specification
Qualifications
* Degree in Physiotherapy and registration with HCPC.
Experience
* Evidence of clinical placements as a student and ability to apply skills across clinical areas.
* Previous experience as a Band 5 physiotherapist (desirable).

* Knowledge of physiotherapy in relevant area.
* Forward thinking and problem‑solving.
* Excellent interpersonal skills.
* Ability to work independently.
* Ability to pass on skills and knowledge to others formally and informally.
* Communication skills for patients with hearing or cognitive difficulties.
