We’re looking for an enthusiastic, proactive and innovative Senior Cluster Co-ordinator to lead the Cluster Support Team within Central Integrated Health Community (IHC), supporting the development of Clusters across Conwy and Denbighshire. This is a fantastic opportunity to be at the forefront of Cluster development in North Wales, where you’ll be working with a diverse range of stakeholders from across health, social care, the third sector, and wider public services. You’ll be supported by a capable and experienced team of Cluster Co-ordinators, insightful and engaged Cluster and Collaborative Leads, and excellent senior leadership within the Primary Care Management Team. Main duties of the job You’ll be responsible for leading and co-ordinating the development of local Cluster plans and ensuring alignment with national policy drivers such as A Healthier Wales and the Primary Care Model for Wales. You’ll work directly with Cluster Leads and Professional Collaborative Leads - including senior healthcare professionals from General Practice, Optometry, Community Pharmacy and Community Nursing - supporting them in their Cluster leadership roles and helping shape Cluster priorities in response to local population need. Strong partnership working is key. You’ll be someone who enjoys building great relationships, communicates with clarity, and brings people with you. You’ll be passionate about whole-system working, able to navigate complexity, and comfortable working across organisational and professional boundaries. You’ll help ensure that the work of the Clusters makes a meaningful contribution to national priorities and leads to real improvements in people’s lives locally. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply. Person specification Qualifications Essential criteria Evidence of masters degree or experience of working to that level Post-graduate management qualification or equivalent experience Desirable criteria Prince2 or equivalent Experience Essential criteria Significant experience of NHS or Social Care management or equivalent Proven experience of development and presentation of reports / information to inform management decisions. Desirable criteria Experience of working across health, social care, voluntary and independent sectors. Previous experience of needs assessment and/or service mapping. Skills Essential criteria Excellent analytical and information gathering skills. Ability to present and explain results of analysis both written and verbal Ability to interpret, synthesize and process large amounts of highly complex information and data to draw conclusions taking into account the underlying context Desirable criteria Ability to speak Welsh or a willingness to achieve an appropriate level of competence. Knowledge Essential Criteria Good understanding of the heath and / or social care management environment in Wales, and the roles and responsibilities of primary and community care Knowledge of the current national and local community and primary care services Desirable criteria Knowledge and understanding of health and social care data sets, information standards and processes Personal Attributes Essential Criteria Ability to develop and sustain personal relationships with a wide range of individuals and within groups Ability to work as part of a team Flexible approach to work Self-directed and a completer / finisher Other Essential criteria Able to work across sites if/when necessary and ability to travel between sites in a timely manner
