Nottingham CityCare are excited to announce that we are recruitingQualified SCPHN Health Visitors at various bases across Nottingham City and areoffering a starting salary of mid-point band 6.

Nottingham CityCare is a dynamic organisation, commissioned to provideNHS services in the community.

We are currently recruiting qualified Health Visitors that are committedto developing and embedding innovative ways of working with key partners todeliver quality childrens public health services.

You will be part of our 0-19 Public Health team, with your caseloadrelated to your specialism (0-5). You will work in various community settingsand clients homes, leading on delivery of the 0-19 Healthy Child Programme viaour universal and targeted services.

Our starting salary ismid-point band 6 for all staff who are newly qualified or have qualified withinthe last three years. Salary can also be negotiateddepending on experience and current rate of pay and we honour agenda for changeTerms and Conditions.

In addition, you will have access to a preceptorship programme which isthe first in the Midlands to have been awarded the National Quality Mark, withprotected time for continued professional development and study sessions.

We also have a dedicatedSafeguarding Service who will provide you with safeguarding advice, support andsupervision. Our supervision package also includes management supervision andclinical/restorative supervision when required


Main duties of the job

In partnership with our partners, you will be delivering our offer alongside evidence-based programmes of care to children and families. You will also lead on reducing health inequalities and improving the health and wellbeing of children and their families within Nottingham City.

You will have:

* Excellent knowledge of the current developments in health visiting and have a proven track record of continuous professional development and innovative practice.
* Excellent communication skills: (both written and verbal), coupled with good IT and organisational / prioritising skills, with the ability to work within a team setting as well as independently.
* Positive outlook and responsive to changes in the future delivery of health care.
* Knowledge and experience in addressing safeguarding issues and participating in safeguarding processes and pathways.

Within the wider service you will also be involved in audit, development of practice, support and supervision to other health care professionals and students.

The hours of work will be minimum 22.5 over 3 days (Part time) to 37.5 hours (Part Time/Full time). The Service hours are Monday Friday 8.30am 5pm excluding weekends and bank holidays.

Job responsibilities

Job Purpose

* To lead and co-ordinate the delivery of the Healthy Child Programmewithin a 0-19 integrated public health team.
* To manage an assigned caseload and be responsible and accountable forthis caseload
* In partnership with the Early Help team, you will be deliveringevidence-based packages of care to give Nottingham children the best start inlife.
* To signpost families to other services to ensure that children, youngpeople, and families have the support they require to meet their individualneeds.
* To support the Clinical Service Leads and Managers to deliver anintegrated service offer for children and young people aged 0 - 19 and theirfamilies.
* To work collegiately with other key partners to ensure the delivery ofhigh-quality childrens services throughout Nottingham City and across the 019 age range.
* To provide appropriate care to children, young people, and theirfamilies in line with professional registration.

Dimensions

The Health Visitor will be responsible for:

* Staffmanagement, including all aspects of human resource policy and procedure.
* Financialmanagement, to work in a cost-effective manner.
* Work inpartnership with other key partners such as Early Help to deliverevidence-based programmes of care tochildren, young people, and their families in Nottingham city in a variety of communitysettings.
* Supportservice improvement by leading and/or participating in corporate and local workprogrammes, projects, workshops, or other initiatives; and supporting the implementationof new working practices.
* Maintainown mandatory and role specific training and deliver training / group work tostaff and clients.

Key Responsibilities

Professional

* To work within the NMC Code of ProfessionalConduct.
* To take a lead role in our SmallSteps Big Changes (SSBC) programme to improve the outcomes of 0-3 years oldthrough a universal enhanced offer of support in 4 wards in Nottingham city.
* To take a lead role for aspecialist area of practice within the team.For example, Safeguarding, Infant Nutrition, SystmOne, etc.
* To take a lead professional rolewhen required where children and young people with additional needs require anintegrated package of support from more than one practitioner/service.
* To actively engage in thereflective supervision process, to ensure that expertise in clinical methods isdeveloped and maintained and that the service offered is high quality.
* To work in collaboration with localstatutory and mainstream services to ensure that families become confident inaccessing a range of other universal health and support services.
* To contribute to the development ofidentified public health initiatives.
* Use high level interpersonal andcommunication skills to provide and receive complex and often highly sensitiveinformation. Use strengths based, solution focused strategies and motivationalinterviewing skills to enable families to develop behavior change strategies.
* Demonstrate effective managementand leadership skills
* Anticipate and reduce barriers tocommunication.
* Enable children, young people, andfamilies to express their anxieties and concerns and, where appropriate, act asan advocate for the service user who may require support in accessingappropriate services.
* Be responsible for and able toassess and interpret complex written and verbal referrals/reports.
* Providing consultation to the teamon identified children, young persons or families and advising on appropriatecare or referral.

Clinical

* Plans, develops, implements, andevaluates health improvement programmes with babies, children, young people,and families.
* To use evidence-based programmes, materials,and methods in a range of settings to improve the outcomes of pregnancy andimprove childrens health and development.
* Ensures the universal delivery ofthe Healthy Child Programme and plan and co-ordinate clients care inconsultation and collaboration with other professionals.
* Take the role of the leadprofessional in delivery of care utilising the Common Assessment Framework /Priority Families model.
* To work therapeutically, using arange of skills including prescribing, with a range of family members topromote behavior change and positive outcomes for children and families.
* Develop therapeutic relationships, requiringin-depth mental attention and concentration for extended periods, and promoteadaptive behavior change in the family home and other environments.
* To provide advice, signposting and support forparents, carers and young people using Text Health, our text messaging service.
* Use professional and clinicaljudgement to undertake detailed assessments of clients and their families andanalyse complex family situations to develop appropriate clinicalinterventions.
* Responsible for complex informationgathering, from a wide range of sources, some of which may be conflicting.
* Works effectively with a wide rangeof diverse people to identify their health and wellbeing needs.
* Work with partners especially inEarly Help teams in Children Centres, GPs, Midwives and Social Care wherefamilies have ongoing needs requiring multi agency support.
* Actively engage and work withfathers and other members of the extended family to achieve positive outcomesfor children and families.
* Promote community capacity buildingto enable families and communities to build on their strengths to provide newways for improving service.
* Integrate into practice emergingtheories in neuroscience and early intervention to effectively support familieswith complex needs.
* At all times keep the safety andwellbeing of the child in mind and work within local safeguarding policies andprocedures.
* To undertake additional trainingand complete competency packages to deliver enhanced clinical roles e.g., immunisations,newborn bloodspot screening etc.

* Keep accurate electronic and paperrecords consistent with the current record keeping policy.
* Ensure robust communication systemsexist between key stakeholders including, general practice, education, socialcare, public health, and voluntary services.

Qualifications

* Registered Specialist Community Public Health Nurse (Health Visiting/School Nurse).
* Mentorship Programme, e.g., supporting practice learning or equivalent
* Non-medical prescribing (V100 or V300)
* Evidence of further professional development including-
* Dip Level 2 Modules in Family Planning
* Teaching & Assessing, Counselling


Experience

* Evidence of working with diverse or disadvantaged groups.
* Experience of working with children, families, and young people.
* Experience of working in partnership with key partners to safeguard children, young people, and vulnerable adults.
* Experience of clinical supervision.
* Evidence of delivering health promotion activities to groups.
