Portage, Quality and Inclusion Practitioner
An exciting opportunity has become available to work within Best Start in Life Portage Quality and Inclusion Practitioners team within the Family Hubs. The position is for 2 FTE (37 hours) per week working Monday to Friday. One will be based in the North locality and one in the South locality.
About you
1. Support Early Years Settings (nurseries, preschools)
1. Help staff deliver high-quality inclusive practice
2. Advise on adapting activities for children with additional needs
3. Model strategies and interventions (e.g. communication tools, visual supports)
2. Work Directly with Children & Families
4. Carry out holistic assessments of children and family needs
5. Monitor children’s progress and plan next steps
6. Liaise closely with parents, carers, and professionals
3. Deliver Portage Home Visits
7. Provide structured home-based learning sessions for children with SEND
8. Support development through play-based activities
9. Help families build confidence in supporting their child
4. Early Identification & Intervention
10. Spot children not meeting developmental milestones
11. Put in place support strategies early
12. Use the graduated approach (assess–plan–do–review)
5. Multi-Agency Working
13. Work with professionals such as:
14. Health visitors
15. SEND teams
16. Speech & language therapists
17. Ensure families get joined-up support
6. Support Family Hubs & Parenting Programmes
18. Help deliver inclusive parenting programmes
19. Guide families to the Local Offer and support services
7. Safeguarding & Compliance
20. Follow safeguarding procedures
21. Ensure children and families are kept safe
22. Work within policies (EYFS, SEND, GDPR etc.)
