Job Title: Social Worker – Pathway Integrated Team – Westminster
Location: Westminster
Pay Rate: £32.55 per hour
Liquid Personnel is seeking an experienced Adults Social Worker to join the dynamic and fast-paced team. This is a vital role that supports individuals across the tri-borough area, ensuring timely interventions and high-quality care planning.
Responsibilities:
* Conduct urgent home visits within the community.
* Complete Care Act Assessments and develop Support Plans.
* Update detailed and accurate case notes.
* Undertake Mental Capacity Assessments and make best-interest decisions.
* Collaborate with the Home First Team multidisciplinary team (MDT).
* Carry out safeguarding initial enquiries.

Qualifications and Experience :
To be successful in this role you must have,
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK driving licence
* Post-qualified experience
* Degree level or equivalent in Social Work
