Disabilities and the wider Learning Disabilities Team, you will act as a visible clinical expert, advisor and advocate, championing reasonable adjustments, reducing health inequalities, and ensuring our services meet national best practice standards.
In this role you will provide direct care, advice, education, training and system improvement to patients, families, carers and clinical teams. Based at Newham Hospital you will work across Barts Health to build strong partnerships with colleagues in acute, community, social care and voluntary services, and contribute to local and national learning and innovation through initiatives such as the LeDeR programme and the NHS England Learning Disability Improvement Standards.
Responsibilities
* Deliver ongoing expert clinical input for complex patients with learning disabilities (LD) and autism across all sites and all departments within Barts Health.
* Offer clinical teams in all settings expert advice, support and education to make personalised reasonable adjustments and promote the use of the Hospital passport.
* Plan, organise and coordinate the admission and discharge process for complex patients.
* Deliver training to all grades and backgrounds of staff to support awareness of the needs of patients with a learning disability and autism.
* Work with patient advocacy groups in the community, family and carer groups to obtain ongoing feedback for improvement of hospital services in the care and treatment of people with learning disabilities and autism.
* Lead on regular audit and quality improvement initiatives; conduct research with multidisciplinary colleagues.
