Job Summary
18 hours per week. Join a dedicated team providing out of hours district nurse cover in patients' homes across Plymouth, South Hams, and West Devon. This part-time role (18.0 hours per week, 17:00 – 23:00) is ideal for an experienced and motivated nurse eager to contribute to a growing Urgent Care Nursing service.
You will deliver high-quality, patient-centered care in the community, helping individuals remain safe and independent at home while supporting their health and wellbeing. The role involves utilising clinical skills and the administration of intravenous therapy (training will be given).
We seek a nurse with strong clinical knowledge, sound judgment, and the ability to work autonomously and collaboratively in a forward-thinking team. The post requires working out of hours and autonomous working with initiative and passion, supported by sound clinical knowledge and judgment.
All Livewell Southwest staff are expected to be able and willing to work across a 7-day service.
Travel across Plymouth and other geographical areas is required. A full driving licence and access to a car for work are essential; pool cars are available. Reasonable adjustments under the Equality Act will be considered.

Main Duties of the Job
You are responsible for the delivery of high-quality evidence-based care for acutely ill patients within their homes, liaising with the referring clinician and supported by senior clinicians and a clinical manager. You will assess, plan, implement, monitor and evaluate patient care, aiming to prevent hospital admissions and facilitate rapid discharge.
Coordinate shifts in the absence of a senior clinician. The Urgent Care Nursing team operates on a flexible rota; the service covers 24/7, with this post delivering a 6-hour shift from 17:00 to 23:00. You will interface between hospital, primary care, community and social care settings.

Knowledge
* Essential: Background knowledge of nursing and health care; government policies relating to acute care in the community; understanding of clinical governance; clinical supervision and CPD; understanding of team function; overview of acute discharge process and community nursing; up-to-date knowledge of the NMC Code of Conduct.
Skills
* Essential: Ability to respond to urgent calls in different parts of the city; strong acute care skills; autonomous practice; clinical risk management; infection control; acute care assessment; excellent communication and interpersonal skills; computer literacy; time management; delegation; leadership and coordination; intravenous/subcutaneous/central line management; ability to discuss complex patient care with senior clinicians; flexible and capable of prioritising; patient/carer advocacy.
Desirable
* Knowledge of secondary and primary care sectors; wound care; triage experience; respiratory or cardiac background; venepuncture and cannulation.
Qualifications
* NMC Registered Nurse (Adult); current UK registration. Valid driving licence and car access required. Reasonable adjustments under the Equality Act will be considered. Desirable qualifications include teaching/mentorship at degree level or equivalent non-medical prescribing.
Experience
* Essential: Experience in clinical assessment; experience or understanding of community working; experience in a variety of healthcare settings.
