"An exciting opportunity has arisen to support the Hospital Discharge Team to improve the discharge process and support flow through the organisation. We are looking for a motivated and passionate person to join our team. The post holder will be based within the Hospital Flow structure, working within the acute setting across BHRUT Hospitals. They will be responsible for supporting discharges within inpatient wards. They will be expected to work closely alongside the Multidisciplinary Team (MDT), patients and families, our Integrated Discharge Hub, and community partners to ensure smooth and safe discharges. They will join the existing team of Discharge Coordinators to support the planning of timely and appropriate discharges, working to support the Trust goal to increase the number of discharges taking place before 12 noon. The successful applicant will have contact with patients and service users. Main duties of the job
The role will involve focusing on patients that no longer meet the criteria to reside in an acute bed. They will work as part of the ward team to focus on early identification of discharge planning and potential complex needs. They will be supported by the wards and the Hospital Discharge Team. This role may include weekend working.
They will be expected to support the delivery of the Trust’s strategic objectives of increasing hospital flow, by achieving earlier discharges, reducing overall length of stay and having oversight of and challenging factors that would potentially delay discharges."

These CDCs are open 12 hours a day, 7 days a week and are providing an extra 94,000 scans a year.

Essential criteria
1. Good basic education to GCSE or equivalent level with an A-C grade in English and Maths.
Desirable criteria
2. Evidence of continuous personal development.
Experience/ Knowledge

3. Understanding and experience of delivering patient centred care.
4. Understanding of acute hospital performance and importance of patient flow.

5. Knowledge of current Government discharge policy and pathways.
