Integrated Discharge Team – Maternity Cover (Band 6 Assessor)
The Integrated Discharge Team currently has 1 post available for maternity cover for 1 x band 6 assessor at Basildon hospital to cover until 24/05/2027.
Key Responsibilities
* Assess patients that are medically optimised for discharge to ascertain their care needs and make referrals for ongoing patient care in the community.
* Facilitate a safe and effective discharge from the acute trust in a timely manner.
* Communicate effectively with patients, relatives, the MDT and the wider system partners to maintain good patient flow.
* Provide guidance and professional support to patients and their families, working in partnership to develop a discharge plan without duplicating interventions.
* Support patients with complex healthcare needs, including long‑term health and social care planning or short‑term interventions following a change of condition or acute illness.
