Overview
Community Specialist Speech and Language Therapist role at The Mid Yorkshire Hospitals NHS Trust. The post involves working across the Wakefield and district community, visiting patients in Nursing and Care homes, private homes and in clinics. The service supports a wide range of caseloads including stroke, brain injury, progressive neuro, dementia and respiratory, addressing swallow and communication difficulties. You will work alongside skilled and supportive Community SLT colleagues.

Main duties
* Provide a high-quality speech and language therapy service in community settings for patients with swallow and communication difficulties.
* Demonstrate highly developed skills in assessment, diagnosis and treatment of stroke, brain injury, progressive neuro, dementia and respiratory presentations, among others.
* Manage your own clinical caseload and contribute to audit and service development projects.
* Support and trainband 5 SLTs with supervision and fellow staff development.
* Deliver training and education to patients, carers and third parties.
* Liaise with colleagues in acute and community settings to facilitate smooth transfer of care and support early discharge and reduction of unscheduled admissions.
* Supervise, support and provide clinical education to less experienced SLTs, students, and assistants.
* Lead relevant projects for the SLT service.
* Demonstrate excellent interpersonal, communication, motivational and team-working skills, and emerging leadership.
Key requirements
* Degree in a relevant therapy profession.
* HCPC registration as a Speech and Language Therapist.
* Evidence of ongoing CPD; ability to demonstrate how skills, knowledge and experience meet the job description requirements.
