Responsibilities

* Carry out comprehensive clinical assessments and physical examinations
* Diagnose, treat, and manage patients independently, including prescribing medication
* Refer patients to appropriate diagnostic tests or specialist services when needed
* Collaborate closely with GPs, nurses, paramedics, social care, and other professionals
* Lead on safeguarding, emergency situations, and complex care needs
* Contribute to service improvement, audits, and clinical governance
* Provide leadership, mentorship, and training to clinical and non‑clinical colleagues
* Maintain accurate, high‑quality clinical records in line with best practice and policies

Job Description / Responsibilities

The Advanced Nurse Practitioner will be responsible for the assessment of health and clinical needs of the patient. To undertake consultations with patients as an autonomous practitioner using advanced clinical practice skills and clinical judgement to diagnose, treat, refer or discharge patients.

* Undertake consultations with patients taking a comprehensive patient history and carry out physical examination
* Use expert knowledge and clinical judgement to identify potential diagnoses
* Exercise a high degree of autonomy with advanced clinical practice skills to include clinical interventions
* Refer patients for investigations when necessary
* Make a final diagnosis and decide on treatment, including prescribing medicines or referring to primary or secondary care services
* Ensure treatments and care are based on best practice and evidence‑based care
* Plan and provide skilled, competent care involving other members of the healthcare team as appropriate
* Work with multidisciplinary and multi‑agency teams to meet patient needs
* Assess and evaluate the effectiveness of treatment with the patient and adjust as needed
* Use independent/supplementary prescribing skills for patient benefit
* Work independently and as part of a team, providing support, leadership, and training where required
* Assist in clinical audits and monitor service delivery and unscheduled care
* Identify and address risks to patients, including emergency care needs requiring 999 dispatch
* Safeguard children and protect vulnerable adults as required
* Maintain contemporaneous records in line with NMC guidance and local policies
* Communicate effectively within the team and with external organisations; use critical thinking to assess cultural, social, and economic factors
* Utilise interpreter services and communication aids to address barriers
* Contribute to education, training and development, clinical supervision, and mentoring
* Participate in quality improvement activities, audits, incidents, complaints and governance activities
* Adhere to organisational policies, professional codes of conduct, and governance requirements


Qualifications & Experience

Essential

* Independent/supplementary prescriber
* Evidence of study to conduct paediatric clinical examinations
* Teaching/training qualifications (e.g., ENB998 or equivalent)
* Minimum 5 years post‑registration experience
* 2 years experience as a practitioner in a primary care/urgent care setting
* Experience in mentorship or coaching to other staff
* Masters level study in advanced practice (or equivalent)
* Independent/supplementary prescribing
