Charlton Hill Surgery are seeking to employ an enthusiastic, dynamic, and experienced Primary Care Paramedic tojoin our expanding Healthcare Team. Experience of working in Primary Care is essential and applicants who do not meet this criteria will be rejected.
Main duties of the job
Triage and treatpatients making any necessary referrals to other members of the organiseadmission if required.
Assessing andtreating patients presenting with minor or acute illnesses
Promoteself-management where appropriate, including signposting patients to othercommunity or voluntary services.
Ensure clinicalpractice is safe and effective and remains within boundaries of competence, andto acknowledge limitations
Support with Auditsand long term conditions.
As this role is to work core hours Monday to Friday between am and pm there is no unsociable hours pay. Therefore if you are currently working shifts although our hourly rate will be generous we may be unable to match your current 'package'

Charlton Hill Surgery is a large practice with15,100 patients. We have a diverse multi-disciplinary team with 12 GPs, 4Nurses, 1 HCA as well as other allied colleagues such as Physician Associates, First ContactPhysiotherapists, Health & Wellbeing Coaches and Clinical Pharmacists. Weare a training practice hosting junior doctors and medical students and wewould encourage the post holder to become involved in this as part of the ethosof the practice.

Job responsibilities
To work within thecommunity, as an autonomous, accountable Paramedic and provide holistic carefor individuals including assessment, diagnosis and management.
Assess and triagepatients, including same day triage, and as appropriate provide definitivetreatment (including prescribing medications following policy, patient groupdirectives, NICE (national) and local clinical guidelines and local carepathways) or make necessary referrals to other members of the team.
To refer and/orsignpost patients in their own home or care home to other allied healthprofessionals for example community COPD nurses, Social prescribers etc.
To be able to; -perform specialist health checks and reviews - perform and interpret ECGs -perform investigatory procedures as required - undertake the collection of pathologicalspecimens including intravenous blood samples, swabs, and other samples withintheir scope of practice, and within line of local and national guidance;
Support thedelivery of anticipatory care plans and lead certain community services (monitoring blood pressure and diabetes risk of elderly patients living insheltered housing).
Provide analternative model to urgent and same day GP home visits for the network andclinical audits.
Maintain accurateand contemporaneous health records appropriate to the consultation, ensuringaccurate completion of all necessary documentation associated with patienthealth care and registration with the practice.
Advise patients onmaintaining health and refer to allied services when a health promotion need isidentified. Communicate at all levels within the multidisciplinary teamensuring an effective service is delivered.
Communicateeffectively with patients, and where appropriate family members and theircarers, where applicable, complex and sensitive information regarding theirphysical health needs, results, findings, and treatment choices.
Take responsibilityfor safeguarding children and vulnerable adults and work with other teammembers to ensure they are protected.
Work closely withthe multi-professional, management and administration teams, reporting anyproblems encountered to the relevant person and ensuring everyone is aware ofthe different roles within the team. Work closely with the wider PCN andCommunity teams.
It will benecessary to attend and contribute to various MDT meetings as requested.

Qualifications
Essential
1. Educated to degree/diploma level in Paramedicine or equivalent experience
2. Health & Care Professions Council (HCPC) registration
3. Has completed and been signed off formally within the clinical pillar competencies of the paramedic FCP/AP roadmap to practice
4. Is working as part of a rotational model, in which they have access to regular supervision and support from clinicians signed off at clinical practice masters level or equivalent.
5. Full UK driving licence

6. Experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans
7. working knowledge of Microsoft, GP practice and prescribing & data monitoring systems
8. Ability to write comprehensive clinical notes, implement and evaluate care plans
9. Be aware of data protection (GDPR) and confidentiality issues
10. Clear communicator with excellent interpersonal and organisational skills
11. Ability to influence and persuade others, articulating a balanced view and able to constructively question information.
