In this community role, you will work autonomously as part of a team managing a caseload of patients with complex palliative needs. The role includes being part of a large team providing on-call cover for UCLH hospitals, with opportunities to rotate to hospital teams.

1. Act as an expert clinician for patients and families with complex palliative needs, serving as a key worker where appropriate.
2. Manage a caseload of patients with complex needs independently, including referrals.
3. Assess referrals, prioritize needs, and develop treatment plans, supporting junior staff as needed.
4. Provide palliative care advice within multidisciplinary teams, even for patients not currently under palliative care.
5. Deliver clinical interventions focused on quality of life, prognosis, and end-of-life decisions within an ethical framework.
6. Serve as an expert on pain and symptom management, collaborating to enhance palliative care skills across teams.
7. Assess and support bereavement needs of informal carers, making appropriate referrals.
8. Work autonomously in diverse community settings, considering socio-economic and cultural factors.
9. Lead responses in crisis situations, including family conflicts, emergencies, and professional distress.
10. Utilize knowledge of multidisciplinary roles and external agencies to ensure optimal care pathways.
Our service provides palliative care to patients in South Camden & Islington and UCLH. The community team, based at St Pancras Hospital, works closely with the UCLH hospital team. Over the past 25 years, the team has evolved from a small unit to a multi-professional team of nearly 30 staff, including CNSs, consultants, doctors, OTs, social workers, physiotherapists, and administrative staff.

Key responsibilities include:
* Being professionally and legally accountable for all work, adhering to standards and guidelines of the NHS Trusts, NMC Code of Conduct, and HCPC.
* Conducting specialist assessments of patients with complex needs through subjective and objective evaluations.
* Prioritizing care based on advanced clinical reasoning, considering physical, psychological, spiritual, ethical, social, and mental health factors.
* Identifying vulnerable patients and carers, following policies for safeguarding vulnerable adults and children.
