We are looking for an enthusiastic Community Psychiatric Nurse to join our friendly team at Trafford North CMHT. This wide‑ranging role offers a wealth of learning opportunities.
The desired candidate will have proven experience working in a demanding mental health setting and awareness of delivering community mental health care. You will hold and manage a caseload of service users all with SMI and have Care Coordinating responsibilities under the Care Program Approach Guidelines.
You will have proven experience of integrated working with inpatient units, HBTT, social care sector, police and third sector providers. A knowledge of implementation of the MHA within community settings would be beneficial, along with an understanding of provision of care packages and associated documentation.
The team covers an ethnically and culturally diverse area and you will have the opportunity to work with clients from a wide range of backgrounds and psychosocial needs. By assessing your clients and their carers’ needs you will be able to assess and sign‑post to a wide range of community resources and provider services available locally.

Job responsibilities
* Be a full member of the multi‑disciplinary team (MDT).
* Effectively manage your own caseload, accepting referrals from agreed agencies in accordance with operational procedures.
* Undertake formal mental health and social care (Care Act) assessments of new clients and duty system assessments.
* Assess, care‑plan, provide interventions and evaluate, recording community mental health and social care delivered to clients.
* Provide effective care coordination to an identified caseload of complex service users under the Care Programme Approach (CPA).
* Provide specialist mental health assessments, continuing care assessments and advise on management plans as requested by local authority social workers, medical, nursing staff or CCG nurses.
* Promote positive mental health and physical care to clients in all situations and encourage self‑help and independence.
* Support health promotion and act in a consultative role to other community services/agencies.
* Take the clinical lead for the service within your area of specialism, providing advice, guidance and support to colleagues and other professionals.
* Undertake specialist assessments when required in line with the Mental Capacity Act 2007, the Mental Health Act and local authority safeguarding adults procedures.
* Record comprehensive risk assessments and implement risk management plans for clients at risk of vulnerability, self‑harm or risk of harm to others, using the STAR V 2 Risk Assessment Tool.
* Take a lead role in service developments within your area of specialism.
