The Opportunity
Are you passionate about supporting people with mental health problems to maintain their independence, recovery, health, and wellbeing? If so, Platfform would like to hear from you. We are looking for Case Worker in our Neath/Port Talbot service, which includes homelessness prevention services.
This is an exciting opportunity to join a dedicated, passionate team.
We support flexible and remote working arrangements.
Based in Neath/Port Talbot, you will work across both our homelessness prevention and floating support services.
Homelessness Prevention
You will work with individuals who are homeless, at risk of losing their tenancy, and experiencing mental health difficulties. Your role will involve supporting them to secure housing and prevent homelessness, including providing benefit and debt advice, facilitating access to long-term support, and helping improve mental health.
Floating Support
You will provide support to individuals in their own homes, helping them maintain their health and wellbeing to prevent tenancy loss. This includes ensuring benefits are in place, managing rent arrears, and addressing tenancy issues to avoid housing crises.
