This is an exciting opportunity to develop your assessment skills and gain knowledge in a new field of healthcare. The role will involve assessing, reviewing and case managing clients with regards to their health needs, welfare and provisions of care, whether in a domestic setting or
residential placement. Our team is based in the heart of Bristol covering the BNSSG demographic across South Gloucestershire, Bristol and North Somerset.
Main duties of the job
We are work flexibly to undertake face to face assessments for our target population. We deliver person centred assessments of need and subsequent provision of health care if found eligible under the National Framework for Continuing Healthcare. The post requires the applicants to be fully mobile for the role and hold a full UK driving licence.

The job holder will be involved in care coordination as well as highlighting service improvement and governance issues. An ability to work collaboratively with other agencies is essential and experience of working in such a manner is desirable. As this role may involve working with
clients within other areas of care, such as learning difficulties or mental health. A broad range of knowledge and experience is welcomed and encouraged. Training will be tailored to your experience, the role and the needs of the service. Knowledge, Training and Experience
* Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area
* Further training or significant experience in project management, financial management or supporting change management processes
* Experience and understanding of evaluating and measuring the performance of health services.
* Experience in communications and stakeholder management
* Clear communicator with excellent writing, report writing and presentation skills; capable of constructing and delivering clear ideas and concepts concisely and accurately for diverse audiences
