About the role
We are looking for an enthusiastic, motivated individual with a background in nursing, mental health, allied health, or relevant experience managing complex caseloads to provide holistic advice and support to adults who frequently use urgent care services in West Kent.
Our friendly team of health practitioners has extensive experience supporting adults, children, and families through life’s challenges. You’ll join a nationally recognised team that is passionate about their work and supportive of one another.
This service provides strong support for people who frequently access health services, including A&E, primary care, and mental health services.
This exciting role within a successful service offers the chance to be creative and influence how services are delivered.
Responsibilities
The role involves identifying the unaddressed medical, psychological, and social needs of patients who frequently use urgent care services. Needs will be met through different methods such as listening to patients’ concerns without judgment, de‑escalating crises related to physical or mental health, and acting as a bridge between patients and community services to help prevent future crises.
You will collaborate in a multi‑disciplinary approach across various providers.
