We offer an acute crisis pathway to improve the patient journey and overall experience of service users, families and carers. The post holder will liaise with internal and external teams to ensure a smooth transition between teams and family/carer liaison where appropriate. Your core hours will be Monday to Friday 09:00am to 17:00pm but every day will be different. You will attend inpatient reviews, undertake interventions on a 1:1 basis such as Dialog + (full training provided), support the CRHT in assessment and treatment planning, attend MDT and professionals' meetings and cover one shift per week on Samphire Ward as Nurse in Charge. We are looking for someone who can build on existing internal and external relationships to ensure that our service users are supported by the right people to optimise their recovery. Working closely with our Discharge Facilitator, you will work proactively with service users to ensure purposeful admissions to the inpatient unit and identify goals and barriers to discharge early on.
