This role is a highly specialised CNS providing expert care, advice, guidance and professional judgement within Venous Thromboembolism (VTE) and Anticoagulation for our service users, Trust professionals and external care stakeholders.

Working closely with the VTE/Anticoag Nurse team, Haematology consultants and anticoagulation pharmacist, the CNS provides an anticoagulation dosing and advice service for patients on Vitamin K Antagonists and direct oral anticoagulants across both hospital sites.

This role is eligible for visa sponsorship under the UK Visas & Immigration (UKVI) Skilled Worker route; sponsorship is available only when applicants meet UKVI requirements, including salary thresholds (£25000) and role‑specific criteria.


Main duties of the job

* Provide effective, clear and tailored communication with the patient, their carer(s), the wider team and outside agencies, tailored accordingly, which may be of a complex and sensitive nature.
* Promote and encourage effective communication and liaison with other healthcare professionals with ongoing responsibility for patient care.
* Liaise with multiple agencies within and outside of the Trust and refer on as appropriate to Primary/Secondary care.
* Develop and maintain awareness of the role within the speciality, the organisation and local networks and maintain mechanisms for contact and referral.
* Ensure that appropriate information is disseminated within the speciality, the Trust and external agencies and forums.
* Develop and maintain partnership working with other practitioners, local and national bodies.
* Provide high specialised advice concerning care using appropriate styles and channels of communication to meet the needs of patients, relatives and carers, managers, peers and other professions/agencies.
* Articulate a vision for Trust focus that generates enthusiasm and commitment from both employees and patients.
* Establish and maintain a peer network of support, information and learning with other nurse specialists within the organisation and specialists at a local, national and international level to exchange and enhance knowledge and expertise.
* Vocal open and transparent communication channels that support the teams to do the right thing for people we care for, to be bold when they have good ideas and to speak up when things go wrong.
* Ensure confidentiality and adherence to the Data Protection Act in relation to own practice; maintain full and accurate records of relevant information in patients' medical notes.
* Participate in study days as required to enhance and develop the skills and knowledge required to provide the service.
* Work with consultants, service leads and other key personnel to identify areas that will lead to improved quality of clinical pathways and sustainable workforce/data quality/financial improvements.
* Contribute to improved ways of working leading to improvement of service and data quality standard.
* Responsible for carrying out duties in line with the Trust's e‑rostering processes, including managing rosters, supporting staff allocation, and contributing to effective budget reconciliation and workforce planning.
* Raise concerns regarding adverse patient experience using the appropriate route.
* Practice always within the scope of competency of the practitioner and as assessed by UHSussex.
* Provide clinical assessments of patients using advanced assessment skills and escalating as required.
* Accurately order and interpret a range of blood results, concerning local and national protocols and guidelines, administer medications or prescribe within the individual's scope of practice if a registered non‑medical prescriber.
* Refer patients to other specialist teams as required according to UHSussex and local policies and protocols.
* Ensure a welcoming, caring and supportive approach is afforded to all patients and their families, friends and other health care professionals.
* Uphold quality initiatives that improve customer care and enhance the interface between staff, patients and visitors within the trust.

Person Specification


Qualifications / Experience

* Current NMC Registration Sub part 1 Adult Nurse Level 1 with substantial post‑registration experience.
* Significant experience post registration in Cardiology/stroke/VTE in the acute nursing environment.
* Evidence of autonomous practice.
* Previous anticoagulation and thrombosis experience.
* University accredited anticoagulation course.
* Non‑Medical Prescriber or a desire to train as one; teaching and mentorship training.
* Work within clinic settings.
* Experience of using anticoagulation dosing software such as DAWN.
